Application Form for Issuance of Duplicate I-Card

The application form for issuance of Duplicate I-Card may be downloaded by the parent, filled in and submitted at the School Office with the following requisite fees:

Application Form for Issuance of Bona fide Student & Fees

The application form for the issuance of Fee Certificate, which is generally sought by parents for ITR purposes or getting the education fee reimbursed by their employers, may be downloaded by the parent, filled in and submitted at the School Office. The same shall be issued if and only if all dues of the concerned parent are clear.

Application for Change in Address

The application form for Change of Address/Phone Nos. may be downloaded by the parent, filled in and submitted at the School Office for updating in the School records.

Application for Change in Transport Details

The application form for availing transport facility or withdrawal of the same may be downloaded by the parent, filled in and submitted at the School Office. The withdrawal shall be done with immediate effect and the Transport Commitment Charges deposited shall be forfeited. However, the application for providing transport facility shall be considered after verifying the availability of seats on the concerned route. In case there is a seat the parent shall be intimated about the same.

Application for Issuance of Transfer Certificate

The application form meant for the issuance of Transfer Certificate may be downloaded by the parent, filled in and submitted to the Principal’s PA.
